Verifying Patient Information

  • Set up settings, enter the code provided at Step 1, enter the patient information and collector’s code, and scan the bar code from the swab tube.

How should the specimen tube be labeled?

Write legibly on the specimen tube using a smudge-proof marker (Fine Point Sharpie recommended). Include: FIRST NAME + LAST NAME + DATE OF BIRTH.

Specimen Bag Policy

  • How many tubes can be placed in one specimen bag?

1 kit = 1 bag. Do not place multiple tubes in one specimen bag.

Sending Collected Specimens

How should collected specimens be sent?

  • Use the FLAT RATE EXPRESS SHIPPING FEDEX BOX only. Drop off at a FEDEX location (with printing services) before 5pm.

Collecting Specimen

Are there any specific instructions for collecting specimens?
